Every parent/guardian must fill out and submit this form in order for the student(s) to be issued a device for the upcoming school year.

PAYK12 - This is the link to pay for any device that is checked out, and to pay for damages of said device.

Devices are issued to every student at the start of the new school year. They are taken up at the end of the year to be cleaned and repaired in preparation for the upcoming year.
